What is an RFC form?

Just to make sure you are talking about the right program, it's either SSDI which is a disability program you are eligible for if you paid inot social security or SSI which is a low income support program not connected to disability.

Dx 9/1/2009, IDC, 1cm, Stage IV, Grade 3, mets, ER+/PR+, HER2-
