Hey ladies!

Going for my PS consultation with Dr Lee and Dr Caterson this week and was wondering if you had any other recommendations? Any PS that are okay with radiation on an immediate reconstruction?

 Thanks!!!

that's good to hear.  The woman I spoke with was seeing the other doctor; she said she routinely waits an hour or more.  But I really don't mind the wait all that much, as long as I know they know I'm there and I know that Dr. Tobias will take all the time I need with me without rushing.  I wasn't annoyed, really, I was afraid I'd been forgotten since no one said anything until an hour after the appointment time.   I guess I've gotten used to offices where the staff speaks to me if I've been waiting 20 minutes past my appointment time and periodically until I do get in.  Of course, I could have gotten up and gone to the window and asked about the delay. . .

Hi Everyone -

I had 2 consults this week with PS in Boston - I met with Dr. Winograd at MGH & Dr. Tobias at BI.  Both doctors took their time with me at the consults, I think Dr. Tobias did a better job at explaining procedures though.  Both are very qualified, and both do DIEP surgeries - I have read in prior posts that Dr. Winograd did not do DIEP, but he does them now - however Dr. Tobias probably does more DIEP surgeries.  Dr. Winograd can do the surgery next month where Dr. Tobias is scheduled out until November.  It's a hard decision to make but my gut feeling is to go with Dr. Winograd for a few reasons.  First I feel more comfortable at MGH as this is the hospital most of my family/I use.  Second I really like Dr. Gadd the breast surgeon who would be doing my bmx.  I have not met with Dr. Houlihan at BI (this is who Dr. Tobias recommended to me) but have read great things about her. Third I can get this surgery started a month or more earlier at MGH - I really hate having this hanging over my head as I feel my life is on hold until I get this done.

Hi Lucyleester,

I had Dr Gadd (who is the best) and Dr. Winograd do my BMX with the Diep on the left with node dissection and PMX ( my choice) on the right with alloderm gummy silicone implant . I was in surgery 9-hrs and both surgeons did an awesome job. The nurses I had post op all told me Dr Winograd was the best and Dr. Gadd highly recommends him and as she told me "I work with these guys every day so trust me" which I did . The implant side is a little smaller and flatter but  I am now doing chemo so I'll see if I want any adjustments when I'm done but with a padded bra you could never tell. The post op care @ MGH was very good also. Well good luck with whatever you choose and where as I know it's not easy but the recovery wasn't as bad as I thought it would be.

Glad to hear MGH is doing DIEP.

Winograd is supposed to be a fantastic microsurgeon. Though he wasn't doing DIEPs until recently, he had a great reputation with MS TRAMs. I had the impression that not doing DIEPs at MGH had more to do with politics and a conservative department than anyone's skill level.

He told me they were concerned that DIEP patients would need more post-surgical care than they were set up (at that time) to provide. I always had the impression that he had the skill to do whatever he wanted.

Hi Mrsb45 -

Thank you for replying about Dr. Gadd and Dr. Winograd - it makes me feel I have made the right choice for me.  I have a question for you - why did you do an implant on one side and DIEP on the other?

Hi Panchoandlefty (Stephanie) -

Thank you too!  I have read your previous posts - very informative!  I feel that Dr. Winograd is a great surgeon as well and spoke with him about DIEP and MS Tram.  He told me that my DIEP may turn out to be an MS Tram as I have had surgeries on my belly before - 2 C-Sections, a hystectomy and 2 laproscopies....but that would depend on what he finds when he starts the surgery.  He is sending my for a CAT Scan prior to the surgery to locate the arteries.  He was very up front with me on the surgery and I found Dr. Tobias to be the same too.

Thank you again for all your help - I will keep everyone up to date as things progress.

I don't know either Dr Lee and Dr Caterson, but I also have an appointment with a plastic surgeon on 9/22/09.  I had a mastectomy in March 2008 without reconstruction.  At the time, I just wanted to just have the mastectomy surgery so I could recover quickly.  Now a little over a year later, I am ready to think about reconstruction again.  I am meeting with Dr. Yoon Chun (she is out of the Faulkner Hospital and Brigham & Women's).  Is anyone familar with her?  I am planning on having a mastectomy to my other breast as well and getting implants.  I am worried about the expanders and infection and I am worried that even though my mammograms and MRI's have come back fine on my "healthy" breast, that when they remove it they might find cancer.  Any way, just curious if anyone has had reconstruction with Dr. Chun.

Thanks for listening.

Hi Lucyleester,

I also had a scar on the right from a previous appendectomy and he felt that their wasn't enough tissue to do the diep on both breasts. I had the scan also and told him to do the tram if he didn't think the vessels would be good enough. I had huge vessels on the left but none near the appendectomy scar that would have been good enough to do both.I didn't want to risk an implant on the left because I had radiation to the left breast last year and theirs risk of the implant shifting due to the radiated tissue being stiff. I felt this was the right decision for me . Good Luck with whatever you choose,

Karen,I don't know Dr. Chun but I know she is in the same office as Dr. Caterson.  The whole staff is really nice and the nurses/PA are all great. If you are considering a DIEP you should talk to Caterson -- for implants I'm sure Dr. Chun is more than competent.  Good luck!

Karen, I sent you a PM.  Dr Chun was my PS, and she is terrific!  I could not be more pleased with her results.
