Topic: What are the recommendations for tx if any?

There are more weapons in the arsenal now. This is apparently not a reoccurrence but a new primary. Different type, different breast. I hope that will be in your favor. That would be my first question for your Dr. You've done all this before and I know you don't want to do it again but, you can.
