Mastectomy path report shows "gray area" for IBC ?Hi ibcspouse, Thank you very much for your comments. My daughter has her path report and I'll ask her to make a copy for me. It has been recommended that she have both sides removed, but for reasons that are not entirely clear to me, they decided to do the LM and axillary dissection now, then proceed to radiation as soon as she has healed. The plastic surgeon who saw my daughter along with her surgeon, stated that they would remove the left breast and do bilateral reconstruction about six months following radiation therapy. Because of her BRCA2 status, she will be having an oophorectomy with removal of fallopian tubes, and possibly a hysterectomy. She will also be on hormonal therapy at some point. I just joined the ibcsupport.org website and will post the question per your suggestion. Thanks again for your help. Mastectomy path report shows "gray area" for IBC ?A little history - My daughter was diagnosed with multicentric BC at age 33. She had been having symptoms for three years (!!!) before mentioning them to me. Being a BC survivor myself and having had a recurrence in 2007, I found this news to be very distressing. On questioning her, it seems she finally went in because the pain from her breast was radiating to her back. Her PCP ordered a mammogram. Because of the symptoms she was describing to me, including an inverted nipple, breast pain and possibly a rash, I was very concerned about the possibility that this was IBC. Unfortunately, the mammogram showed multicentric BC with the largest tumor measuring 8cm! My daughter had several tests including an ultrasound, MRI, PET scan and CT scan, and biopsies of the breast and one lymph node under her arm. All biopsies came back showing BC. She also tested positive for BRCA2. Because of the size of the largest tumor, she underwent A/C and taxol chemotherapy first. She completed that and just last week had a right mastectomy with full axillary dissection. The path report showed the large tumor was 8cm x 6.5cm x 3cm. The surgeon drew a pic of the size on the back of the path report, and it is the size of an orange. The report showed 3/20 lymph nodes positive for BC. It also showed diffuse DCIS throughout the breast. It also showed that the nipple was "affected". When asked directly by my daughter if there was any IBC the doctor replied that this was a "gray area", and although her nipple was involved she was going to call that Paget's. My understanding is that Paget's causes itching and crusting of the nipple, something my daughter didn't really have, although her nipple was inverted. Okay, here's my question - would the path report have identified IBC if it was there, and if not is there a specific test or process that determines when it is in fact IBC? Thanks for any light anyone can shed on this. What to expect from BM and full axillary dissection ?My daughter has one more chemotherapy infusion next week, which will complete her A/C and taxol treatments. She will then be having a bilateral mastectomy with a full axillary dissection on one side. She is BRCA2+. Because of the size of one of her tumors she had chemotherapy first to try to shrink it. My own history is that I have had three lumpectomies and a left mastectomy with SNB. I know what to expect with the mastectomies but am wondering about the full axillary dissection. SO, HOW DO YOU COPE???!!!I have the Burt and White's book Binney mentioned above, and would also recommend "LYMPHEDEMA Understanding and Managing Lymphedema After Cancer Treatment" by the American Cancer Society. It has a forward by the ABC News Correspondent Sam Donaldson who developed leg lymphedema after surgery for a malignant melanoma. It covers who is at risk, precautions, treatment options, helpful resources, insurance issues, how to cope, and more! Interview with my daughter's Radiation Oncologist re LEHi everyone, Since I don't post on this web-site very often, here is a bit of background on me. In 1996, I was diagnosed with LBC and underwent a lumpectomy followed by radiation. In 2006, I noted a puffy area just above my left wrist as well as an unusual aching near the crook of my elbow. When I mentioned this to my doctor he took a look and muttered something about there didn't seem to be any infection and did no further followup and had no comments. In 2007, I was diagnosed with a recurrence of LBC following a biopsy and had a lumpectomy. Since the lumpectomy margins did not return clean I opted for a left breast mastectomy with sentinel node biopsy (which I had requested), which was performed on June 8, 2007. Following my mastectomy I raised the issue of possible LE with my surgeon and he ordered a referral to a lymphedema physical therapist, who confirmed that I have mild LE of my left arm. Remember, I first noted symptoms in 2006, 10 years after my original diagnosis. I had a series of visits with the PT and was trained in doing self-massage and I was given a prescription for a compression sleeve and gauntlet, which I wear faithfully. Unfortunately, about two months ago, my 33-year-old daughter was diagnosed with invasive breast cancer with a positive lymph node. She is scheduled to begin chemo on March 10th, then depending on the results of the BRAC genetic testing she will have either a single or double mastectomy. Following this she will have radiation and then hormone therapy. To say we are still in shock about her diagnosis is an understatement. Okay, that takes care of the background ... here is where Stacy's radiation oncologist's question comes up. I went with Stacy to the appointment with her radiation oncologist yesterday and she interviewed me first. I was not surprised by this because the oncologist had done the same thing a couple of weeks earlier. However, when she asked about my LE she seemed mystified that I had developed it prior to my mastectomy. She asked me if any nodes were treated by radiation in 1996 and I said no, only under my left arm. I told her that it was my impression that radiation alone was a risk factor for developing LE, and she countered with "only when lymph nodes have been treated by radiation". If anyone has any thoughts or comments, I'd appreciate it. I think I am confused by this, which wouldn't be surprising given my daughter's recent diagnosis. Question about symptoms my daughter is having - IBC?Thanks for the responses. Here is an update on my daughter - she had a bilateral digital mammogram followed by ultrasound last Friday at the Breast Care Center at SW Washington Medical Center. The radiologist came in and sat down with her and said they see three areas of concern. One is right behind the nipple, which may be tethered to it causing the inverted nipple and a second area which is up a ways from the nipple area. The third is in her right axilla. She will be having biopsies of all three areas this Monday. They have a pathologist on site so they will be able to give her the results at that time. Question about symptoms my daughter is having - IBC?Hi, My 34-year-old daughter sat me down on Friday and told me that for the past four or five months she has noticed swelling of her right breast, pain in the right breast, as well as an inverted nipple. Why she let this go so long is beyond me, but I believe she was probably hoping it would go away, and her comment was "I didn't want to worry you because you already have so much on your plate". Having just had a lumpectomy with unclear margins and eventual left breast mastectomy myself in 2007, I know just enough about the various kinds of breast cancers to wonder if in fact this doesn't sound like it might be IBC. My BC was for DCIS, not IBC. She had just seen her doctor before she told me about all this on Friday, and her doctor did a thorough breast exam on both sides, including lymph nodes in the axillary region. Her doctor (actually an NP), did not find any lumps. She did order a mammogram with ultrasound to follow if needed, which is scheduled for January 22. I have read just enough about IBC to know that it is very important to get diagnosed and begin treatment ASAP. So, my first question is, is January 22 too long to wait or should we push for an earlier mammogram date? It is also my understanding that biopsy is vital in diagnosing IBC and should she be getting that first? Well, you can see I have lots of questions and would appreciate it so much if any of you who have been diagnosed with IBC could help me out. Thank you. I have been putting lotion on my arm when I wasn't wearing my sleeve, but now realize I should be applying it before putting my sleeve on. I also appreciate your comment about the use of lotion being almost as important as the sleeve - who knew!? Thanks again! Lymphedema in only one hand?I really like the soft clay idea.
My LE therapist had me buy a soft Nerf-type ball so that I could gently squeeze it with my affected hand several times each day. I do this while watching tv in the evening. I think this helps my hand.Diane |

tamoxifen and nortriptylineHi ladies, A couple of days ago I was prescribed nortriptyline (Aventyl, Pamelor), as an off-label treatment for severe temporomandibular dysfunction. I am also supposed to begin tamoxifen. I filled both Rx's and then remembered the discussion here about anti-depressants that interfere with tamoxifen. I checked on DNA Direct and did not see nortriptyline, Aventyl or Pamelor mentioned. Can anyone tell me if nortriptyline is okay to take with tamoxifen? Thanks much! |

reboundingHi Kathy, I recently bought a rebounder based on recommendations from this site, I believe. I really enjoy it! It is very low-impact and easy on my joints. It is too early to tell whether it has helped my lymphedema or not, but any movement or exercise is good for overall health, so for me it was a good decision. |

Left Breast Prosthesis Dilemma and LEHi ladies, On June 8th I had a left breast mastectomy. After I healed sufficiently I was fitted for a breast prosthesis. I am a plus-sized woman and wear a D-cup, so obviously the prosthesis weighs about the same as my remaining right side. Trouble is, now that I have been diagnosed with left arm lymphedema I read that heavy breast prostheses should be avoided. Hmmmmmm. How can I avoid a heavy prosthesis if I need to counterbalance with my remaining side? My bra already shifts to the right a bit, even with the "heavy" prosthesis, which is annoying but something I can live with. A lightweight prosthesis would really make this a challenge. I did a bit of reading on the Lymphnotes site and here is a paragraph that jumped out at me - "Avoid a heavy prosthesis. A heavy prosthesis places pressure against the chest wall, but not where you want compression. It also drags on the bra straps. Wearing a light-weight prosthesis can ease this problem. " Since I don't work outside the home at this time, I suppose I can go bra-less around the house, but I wonder if anyone else has this problem and how you have addressed it. Concerned about my compression sleeve ?Hi and thanks for the responses. I showed my arm to the LE physical therapist and she thought the redness was normal and that the bruises were probably from learning how to put the sleeve on. Jennifer - where can I get artiflex cotton or tubular liners? The itching is driving me crazy. The itching is in two or three places that I can't scratch when the sleeve is on! Help! The worst itching does seem to be in an area where fluid accumulates. Thank you. |

Concerned about my compression sleeve ?Hi Ladies, My custom-made sleeve arrived earlier this week and I was finally able to work up to wearing it most of the day yesterday. However, when I removed it last night was surprised to see a couple of small bruises on my arm, and an area around the upper part of my arm that now has a red rash on it. I do find that the sleeve makes my arm itch quite a bit. Any ideas/suggestion? Thank you. |
