Lumpectomy/Radiation Followed by Mastectomy/Reconstruction

I forgot to ask something. I've been reading the other threads about SNB. Is this something I should bring up with my doctor?? My MRI showed the right breast to be clear with no lymph node enlargement on either side, and that only one tumor was found on the left side, which the pathology report after lumpectomy said had invaded some lobules. The tumor was also relatively large 3-3.5cm. No microinvasion, but some Grade 3 present with necrosis. Should I be having an SNB if I opt for mastectomies in my second surgery??

Lumpectomy/Radiation Followed by Mastectomy/Reconstruction

Hi Everyone,

I haven't posted in a long time but I'm back! I got the results of my pathology report and I was absolutely relieved that my Dx was not upgraded. It stayed DCIS, ER+/PR+, but showed some evidence of Grade 3 with necrosis. The only negative was that the margins were less than 1 mm on a large surface of the tissue sample. My doctor/surgeon told me that he is never happy with anything less than 2-mm, and for someone of my age (32) and family history (mother Dx at 45 with Stage IV), he prefers a minimum of 3-4 mm. So I will be back in the OR soon.

I thought long and hard about things and decided that I will opt for a mastectomy. I am very small chested, and a second lumpectomy will probably dramatically change the shape of my breast. Not to mention that even before my first surgery, my surgeon told me there was a 50-50 chance I would lose my nipple. I am convinced it will go with a second lumpectomy.

 I am meeting with a plastic surgeon in 2 weeks, and with geneticists. My doctor/surgeon told me he prefers his patients heal on their own for 6-12 months before reconstruction, but he also told me it is a gut feel and not substantiated by any kind of data. I feel pretty strongly that I want to wake up with implants.

I still have bad days, where I worry about the future and my little 2 year old being without his mother. But as every day passes, I am feeling more and more confident that a bi-lateral mastectomy will allow me to move forward in the best possible way.

I'm keeping my head clear of negativity! And trying to surround myself with friends and fun as I contemplate these difficult decisions.

Lumpectomy/Radiation Followed by Mastectomy/Reconstruction

I was anticipating receiving the results of my pathology report yesterday, and after several restless nights worrying, I showed up to my appointment and the report was not ready! I was very disappointed. So now I am back in waiting mode.

In my case, Kathi, it was really a lumpectomy. I cannot even tell by the shape of my breasts that any tissue was removed. The scar is the only evidence - a quarter circle on my left breast, starting at 12 and turning clockwise to 3, about 1 cm from the nipple. My doctor said that it can be quite amazing how much tissue can be removed from the upper breast without any noticeable change. I just hope he took out enough and the margins are clear!

Turkey was great!

Thanks for all your replies. sam408, I will definitely consider all my options, also given that my scars heal terribly with keloids. I meet with my breast surgeon on Wednesday to get the full pathology of the lumepctomy sample. Hope all goes well. If the margins are not clear, I am leaning towards mastectomy so I will not have to worry about any of this. Happy Thanksgiving to the Canadians! I just stuffed my turkey.

DCIS Very Close to Chest Wall

In one of my discussions with my breast surgeon, he told me that recurrence is still possible (although much less likely) with mastectomy because there will still be about 2-3% of breast tissue left behind. It is impossible to remove it all. I assumed that the 2-3% remaining tissue would be on the chest wall, but, in actual fact, my surgeon corrected me and told me that it is quite straightforward to "peel" the breast tissue off of the chest wall. There is some layer in between the breast tissue and the chest wall that allows almost all of the breast tissue to be removed there. The 2-3% of tissue that can be left behind is towards the outside of the breast, on the sides. I can get more information when I see him on Wednesday. Hope this bit of information helps.

Lumpectomy/Radiation Followed by Mastectomy/Reconstruction

Just wondering if there is anyone perusing these discussion boards who has had lumpectomy followed by radiation, and then at a later time (after radiation), decided on mastectomy with reconstruction?? Whether it was for recurrence or for prophylactic reasons.

I had a LB lumpectomy last week for DCIS, even though I was leaning towards mastectomy due to strong family history (mother Dx at 45 with Stage IV). I wasn't given a whole lot of time to think about my options because within less than 3 weeks from diagnosis, surgery was done. I am waiting for pathology of the tissue sample right now. If the margins are clear, and my Dx remains the same, I will be starting radiation shortly after. If the margins are not clear, I have a second chance to get a mastectomy (uni- or bi-lateral) and skip the radiation.

My doctor was explaining that the breast skin can thicken and change with radiation, making any possible future reconstruction after mastectomy difficult. Has anyone experienced this?

I will be seeing geneticists soon for BRCA1/2 testing, and even if I do not have a mutation, it is possible at some point that I decide to have prophylactic mastectomies. Reconstruction would be important to me, and I am worried about it not being possible or that the radiated left side will not reconstruct like the non-radiated right side.

Does anyone have any experience to share? Thanks.

Dazed, Confused and Terrified

I am in good shape too, and I felt really well a couple days after surgery. The info I got from the nurse is that strenuous activity and lifting heavy weight can interfere with the physical healing process of the breast after surgery. Apparently, no weight above 25lbs should be lifted (and my two year old boy is 32 lbs!). I told her that my emotional healing requires both physical activity and holding my child, and she was ok with that. I guess it's all about judgement.

Was the same scar reopened for the re-excision, or was a separate one made? I am not a vain person, but my scars heal terribly with keloids (which remain a nuisance for many years with friction from bras, especially during sports and activity).

Dazed, Confused and Terrified

I am 32. I had a lumpectomy for a single focus DCIS (Grade 2) located in the upper left qudarant of my left breast one week ago (Sept 29). I entered surgery with a preference for lumpectomy, but told the doctor that if he had to take the nipple (which was a possibility given close proximity of the DCIS), to take the entire breast. And I also told him that if he took the left breast, to take the right one off as well. He seemed surprised by this. But my mother was Dx with Stage IV BC at 45, and died at 50. And I have a two year old boy. Even though I came out of surgery with only the lumpectomy with nipple intact, I still feel it is a matter of time before I have my breasts removed prophylactically. My surgeon (who is the most wonderful doctor I have ever had or met) promised me that anytime I felt ready for that surgery, he would do it.

I didn't have a lot of time to think about things. Exactly 3 weeks after I got the phone call about the biospy result, I was in the OR. This included getting an MRI and all my pre-op tests done (sometimes the CDN health care system does work!). Yesterday, 6 days after my lumpectomy I ran 5k in a BC fundraiser. Probably should not have, and imagine my surprise when I literally ran into my doctor at the finish line. He gave me a big warm hug and kiss, which is better than the trouble I thought I was going to get into! But I did get a call from his nurse this morning telling me (with a laugh) that I should take it easy for 3-4 weeks.

I am waiting patiently for pathology results. I'm scared, no doubt about that, and hope the Dx stays the same as with the biopsy result. I do have a question. If a re-excision is necessary, is it also through general anesthesia? I am thinking long and hard about whether I should go ahead with mastectomy if the margins are not clear on the tissue sample.

DCIS Diagnosis -- Has this happened to you?

I am 32 and I was diagnosed with DCIS in my left breast on Sept 8, 2008. Six months ago, during an ultrasound to investigate some aches in my right breast, the radiologist found something worth biopsying in my left breast. Pathology came back as "benign breast tissue" and I was told to come back in 6 months for follow-up imaging. During follow-up, the same suspicious area was there, so I was sent for my first ever mammogram, and a cluster of microcalcifications was found in a location consistent with the suspicious area. The radiologist conducted a needle core biopsy and then xrayed the specimen, which showed that she successfully captured microcalcifications with the procedure. The result this time came back as DCIS.

Several thoughts continue to go through my head. 1) Was the biopsy done correctly six months ago, and did this DCIS start over the last 6 months? 2) Was the biopsy done incorrectly and how long has this DCIS been in my breast? 3) Why was I not sent for a mammogram six months ago?

 I was always told that, at 32 years of age, my breasts were too dense for mammograms to be useful but I guess the doctor's opinion will change now. I know it is easy to be retrospective and ask "what if...?" and I am trying not to do that too much. But it did bother me when the radiologist stated that a pathology result that says "benign breast tissue" is usually followed up immediately with a second biospy. She explained that the term "breast tissue" means they haven't quite figured out what it is. But I was only told to come back 6 months later.

I have an MRI scheduled tomorrow morning, and I am afraid that the images will show more. My mother was diagnosed with Stage 4 BC at the age of 45 and passed away 5 years later, so a strong history of pre-menopausal BC exists in the family. I am leaning towards a bi-lateral mastectomy even if the right breast is ok, but I do not want to be too radical. All I can think of is my two year old son and wanting to be there for him always.
