Start with the biopsy. With the information gotten from that, your oncologist will help you decide decide the best course of treatment. If it's very ER+, it may be kept under control with Tamoxifen.

My sister had a double mastectomy about a year ago but the doctor is telling me mine is different. The surgeon and oncologist are recommending removal and radiation and tamoxifin. In my mind I say why take the chance and just have the mastectomy. I am 39 and have three young children I do not want to take the chance of recurrence but then on the other hand am I being to radical. I am very confused!!

I would watch for false positives on MRI.  I had 3 'questionable' spots on MRI.  One turned out to be DCIS the two others were benign.  MRI's can't provide any positive diagnosis.  Only a biopsy of the area can.  Yes, it's a pain in the a** to go back in for more procedures, but I think your surgeon is wise to suggest at least an 80% probability the area will not be cancer.
