Calling all ladies who had mastectomies with immediate tissue expanders: How long did it take your incisions to heal? And how "healed" were you before you got your first fill?

On Feb 3 I had a bilateral mastectomy with immediate TE's, 500cc's on each side. One incision was not healing properly so 2 weeks after BMX I had a 2nd surgery. My PS took out 50ccs on the affected side (pressure was too much for incision to heal closed), he cut away some skin that was dying (necrosis) and he re-sutured me.

Since then I have been religiously applying Bactroban ointment, a strip of Xeroform dressing, and a gauze pad on my right side as instructed and also did another round of antibiotics. My next appt. with PS is in 2 weeks. My incision is STILL not completely healed! And I have yet to have any fills.

Has anyone else had delayed incision healing and delayed fills? I am 52 and in otherwise good health (aside from the breast cancer!).

Hi, could not help but reply...I also had double mastectomy Feb. 3rd. My plastic surgeon did immediate reconstruction with permanent implants..Regardless of what I wanted, he went with the full implant (800cc) My original cancer was in the right side. I had a lumpectomy in 2009 and radiation on that side. That was the side they worried about during the reconstruction. That side has healed great. Still looks like I am hiding a watermelon under my arm on that side. My left side is not healing and your problem sounds a whole lot like mine. I have put the ointment and gauze pads until I have bought out Wal-Mart twice...still no improvement. I had additional surgery Feb. 21st and dead tissue was removed and re-stutured...The stitches were too tight and the skin tore away from the site and now I have an open wound with muscle showing....but he says it will heal. It will leave a big scar, but according to him, no big deal. Yeah right, it is not his body. My mother in her own helpful way, tired the ole...you won't be wearing a swimsuit anyway so who cares what it looks like. Yeah I have so much loving support. I am 49 (almost 50) and thought I was in good health. I hope and pray that your incision heals fast and we can go back to whatever was normal for us both.

Pretty much the same thing happened to me.  I had necrosis on both sides...had the incision areas revised two weeks after bmx and nearly had to have it done again.  I had skin sparing bmx and it was explained to me that there is a lot of trauma that occurs to this skin as surgeons try to remove as much tissue as possible...so sometimes the damage is too much to get normal healing.  It took quite a while, but eventually they did heal.  BeverlySue, I too, had an area that stayed open a good while.  But, like your surgeon told you, it eventually closes together.  I doubted that was possible, but mine actually healed and closed even though I was being expanded every week.  Am now scheduled for exchange 3/16, but will probably have to cancel because hubby has to be out of town for work.  Was sure looking forward to some soft ta tas, though.

I had my BMX with sentinel nodes on right, level 1 nodes on left Aug. 31st. Had my first fill 2 weeks later on September 15th. That's pretty fast but I was healing fast. Everyone is different. Some folks have more complications some don't. I didn't heal as well on chemo but now that it's over I'm healing faster. My drain scars are getting smaller and smaller since chemo ended.

Age plays a part in it too. I was 49 this past fall.

I started putting on vitamin E oil 2x a day about a month after the BMX (as my PT recommended).

Same thing here!  I had a SMX on Jan 26th and still not 100%.  I popped several leaks from 2-4 weeks out and now its just the one spot left.  For a week I was on a high protein diet, bactroban and gauze.  Last week I was upped on the protein (almost 100g a day!) and I use a bandaid to keep it moist.  The whole has dramatically closed over the past few days finally!!! But its still a little weepy.

I see my PS in the morning.  He said last week that if its not closed, he will take me to minor procedures, clean it out and suture it.  However its pretty closed now...just dotting the bandaid now.  So I am anxious to see what he has to say.

I havent had a fill yet.  Im almost 3 weeks past due on that one.  I jokingly call my right one "Betty" and I tell my husband that I am ready for Betty to hit puberty!

Ill let you know how I do tomorrow!  Try more protein!

I had my SMX last October 27 and it took a long time for my scar to heal. It blistered, peeled, and got ugly and seemed to stay that way for so long that I got pretty discouraged.

What worked for me was to take vitamin C and really slow myself down - I was pushing myself too hard and expecting too much, too soon. I spent as much time as possible in a loose fitting cotton shirt to allow good air circulation and also kept everything nice and clean. The healing happened from the inside out, so vitamins, plenty of sleep and rest, a healthy diet, and as little stress and worry as possible. I can't remember how long till before my first fill, but quite a bit later than anticipated. I had my last fill today and will have my DIEP on Mar. 22nd. I do have a dark and pinkish scar - almost 'nipple-ish' - mine ended up being kind of a triangle shape, but the scarred area will be removed at the time of my DIEP. You definitely want to wait until your skin is healthy before proceeding with fill. I think my anxiety about my non-healing scar added to the problem - I felt like I had someone failed . . crazy, I know, but that was the way my mind worked. I had to give myself permission to heal at my own pace while doing everything reasonable to promote healing.

I'm 54 - don't know if age is a factor or not. I looked into trying alternative topical ointments but decided to follow my doctor's instructions.

The TE got much more comfortable once I started getting the fills - no more wrinkly edges!

Thanks for your feedback ladies!! It helps to hear that delayed healing is normal healing for some people. I was able to get an earlier appt. with my PS so I'm going to see him tomorrow morning now instead of 3/22. Hopefully he will be OK with my progress...fingers crossed! (I do NOT want another surgery until my exchange if I can help it!)

BeverlySue: Your situation sounds eerily similar to mine. I hope & pray that your incision heals fast too and yes, let's get back to what was normal for us before all this!

Nanadee: Good to hear your incision did finally close but sorry you too had to go through that. I hope you get to keep your 3/16 exchange date!

Lago: Thanks for sharing, I will try Vit E on my bullet holes (that's what I call my drain scars!)

SuebeeBC: I will try more protein .. and please let us know how it goes tomorrow! Good luck!

Treesprite: Will take your advice and try to slooow down. (I still think I'm 25 so am perplexed that I'm not healing as fast as I used to. Sigh.) Also good to hear that the "coconuts" get more comfortable after the fills begin. I can't wait! (See, there I go again with the not-waiting thing!) OK, I guess I DO need to slow down!).

I too had my PS appointment today. He is very happy with how the incision is healing (though it is still not closed and i thought it looked infected but PS said it wasn't).

SuebeeBC, he told me the same thing your PS just told you...keep it moist & covered. I see him in 2 more weeks, but he said he will likely wait 2 weeks after that before he begins any fills. Since he took 50ccs out of my right breast during the revision surgery, he will need to put that back in so they are even, THEN we will go from there.

I was thrilled everything looked good to him! there are areas of redness on both breasts that I thought might be of concern, but he again said nope, everything is healing fine. So I was VERY relieved! (In the back of my mind I was afraid he would say, "OK Missy, we have to wheel you back into surgery!" Thankfully that did not happen.)

Good luck SuebeeBC, you'll have to let us know how it goes on Friday!

Hello, Everyone:

I had my BMX on 09/17/10; I just had my TE's replaced with implants on 01/24/11.  I had necrosis, which was removed in another operation on 10/06, then had to have a deflated TE replaced on 12/06. There definitely were some "bumps in the road" with this surgery; I had hoped to get the implants replaced much sooner than January of this year.

After each surgery, my PS told me I could exercise gently, which meant I could walk at a pace of 2.5 miles/hour, for roughly two - four weeks afterwords.  It just depends on how long it takes you to heal before you can really start exercising again.

Suebee: Just replied to your news on another thread, but Congrats again! Just don't do more than you really should, even if you feel like it.

To wit: I went grocery shopping on Friday for the first time and had the bag boy carry my groceries to the car. (good.) When I got home I carried items practically one at a time into the house. (also good.) But what I didn't think about was pushing the grocery cart! (Not good.) My chest hurt when I got home and I realized I should not be pushing anything at all heavy like that. So if DH can't go to the market (and he does but he doesn't like it), I may start having my groceries delivered. My sister just set that up for my Mom (my mother no longer drives and is dependent on me to take her shopping) and it is working out great for her. Why didn't I think of setting that up for myself? Ergh. Just one more life lesson BC is teaching me!

Rennasus - I had skin sparing/nipple sparing BMX with TE on Nov 1, complete node dissection on Dec. 6, emergency necrotic skin excision on 12/21, another revision (veritcal from the nipple to the intramammary) 1/7 and another on 1/20 that resulted in the loss of the left (prophy) TE.  I am mid-chemo now and won't have the TE replaced until late June.  I can then begin fills and hopefully exchange by the end of the year.  Don't feel like the Lone Ranger.  It will optimistically be 8 months before the start of fills since my BMX.

How much protein are you eating?  i'm aiming for 75 - 100 grams a day. I also heard Bromelain is good for healing, so I am taking that, too.  Anyone else?

SpecialK... thanks so much for sharing! We have had a similar number of surgeries for this and yes, it feels good to know we're not alone!

My PS said if this doesn't heal, the TE will have to come out and we'll try again down the road..

Does your PS think your body is simply rejecting the TE as well? What I don't get is how putting another one in will be any different... do you know?

Rennasus what about a multi vitamin too?  I heard Iron can play a role.

My regimine was a boost and an egg at breakfast (this is all with other normal foods), greek yogurt and berries as a snack, a meat lunch (like a chicken sandwich or burger), nuts and/or a protein bar for a snack and meat with dinner.  I took a multivitamin in the afternoon because the boost shake has vitamins too.

I hope for all of you that it heals quickly.  Mine lasted 8 weeks only and it was an eternity to me.  I can only imagine months on months.  I hope it doesnt come to TE removal.

Rennasus - My PS felt that the TE was being viewed as "a foeign body" by that compromised necrotic skin and the healing was inhibited.  He was right because once he removed the TE it was fine and healed very well.  I have a TE on the other side (cancer side) and I never had a problem with it.  The incision under the breast fold healed with no problem on the bad side it was only the skin on the front of the breast below the nipple.  He is not worried about replacing the TE once I am done with chemo because he will not be doing anything with that healed skin on the breast proper.  He will go back through the intramammary fold incision.  The nature of skin sparing/nipple sparing asks your skin to re-establish blood supply with the muscle and it just didn't happen with the left one.  Happens in about 20% of those cases.  By the time he replaces the left TE and I begin fills the old will have been out for 5 months so that skin is strong now and the blood supply is great.  I just saw him about 2 weeks ago and he is very happy with how everything looks.  Having one TE is making me crazy - I have to wear a fake boob AND a wig in 90 degree weather in Florida, but Hey I love a challenge!

Rennasus:  Checking on you.  Sorry about that darn spot.

As to replacing the TE, I'm not sure how that would be any different.  You may need to wait to have it replaced (like SpecialK).  Hopefully, that won't be the case.

Working through this whole process can be extremely frustrating, with complications.   As Sue says, I hope you don't have to get the TE removed.
