My TX plan was always to do neoadjuvant to get the 'nest' to form into a 'lump' with good margin and to shrink. Then UMX. Then adjuvant to 'clean up' any cells that might have been 'hiding' and missed during surgery. It also needs to be remembered that I am IBC which there aren't many of us ( 1-4 out of 100 DXs of all typres of BC will be IBC). For IBC, the SOP for quite a few years (even before my DX) has been neoadjuvant but neoadjuvant is becoming more common for other types.

I did 4DD A/C neoadjuvant, 2 weeks later UMX, 3 weeks later 12 weekly Taxol, a week later 25 rads.

Yes, sbelizabeth, and I have a BRCA1 variation of unknown significance. BRCA2 men have a one in eight chance of developing breast and/or prostate cancer in their lifetime, while BRCA2 men have something like a one in thirty chance of developing the same cancers.

I too had shrinkage but nothing close to a pcr. My MO told me this was the most likely outcome since I was ER+. He said the reason for neo-adjuvant chemo was to clean out any cancer in the bloodstream that might be ready to cause metastatic disease, and to reduce tumor bulk, especially in my case, in the axillary area. I still had 8 positive lymph nodes when I had surgery. That surgery was 7 years ago this week.

I did not have PCR and was told I wouldn't since I was high ER. I had some response. I had a couple of intramammary and a rotter node that all cleared prior to surgery which was a relief. My BS would not have taken out the intramamary or the rotter which are in the pectoral muscle. The chemo shrunk my tumor a bit and moved me from a 3c to a 3 a. I am happy with that.
