MRI guided Biopsy BI-RADS 4

Hi All, I recently went for my yearly Breast MRI and they found 2 new foci, 1.8cm apart in my left reconstructed breast (DIEP). They have classified it as BI-RADS 4, and are sending me to get a surgeon to do an MRI guided biopsy. I'm obviously afraid it's back. Wondering if anybody has had a recurrence after DIEP in that breast. My original diagnosis was Jan 2021, Neo adjuvant chemo, DX, radiation and DIEP surgeries.

Also, has anybody had an MRI guided biopsy? not looking forward to this at all.

    wildviolette, I’m sorry you’re going through this. I had an MRI-guided biopsy. I admit it wasn’t as easy as an ultrasound but it wasn’t horrible. The worst part for me was the positioning. Once that was done it was okay, bearable.

    yes, im happy to report it showed a fat necrosis behind the DIEP flap! So relieved. Thank you!

    @wildviolette Very sorry I missed your post previously - I had fat necrosis occur twice (once a year after my initial BMX/DIEP operation, before any fat grafting, then 2 years after that once in the opposite recon breast). In both cases I had diagnostic mammo and sono done which confirmed fat necrosis which was what the doctors (PS first time, my GYN second time) also thought it was. I'm just surprised that the radiologist designated it as a BIRADS-4 and had you undergo an MRI-guided biopsy as fat necrosis is pretty easy to distinguish on combined mammo/sono testing according to my doctors. Sorry you had to go through that and all the accompanying anxiety but very glad to hear about the outcome.

    yes, I initially had a breast MRI and they saw two enhancing foci with washout kinetics so they went ahead and did the biopsy. It was deep on the chest wall. She only biopsied one spot, there are two, so they want a repeat MRI in 5 months. It’s a lot of stress and worry for sure. Thanks for your post @abigailj.
