64 yr. Old with clear nipple discharge and scaly red patch on areola
Diagnostic Mammo and US found nothing, but was referred for breast mri. I need an mri that can be done without lying on my stomach, that is causing confusion and delay in getting the mri. I had my yearly dermatology skin check today. She noticed the red scaly red area and wants to biopsy it if it does not go away soon. I started with the clear nipple discharge a couple of months ago, it is in the right nipple and is spontaneous. The scaly area and general itcy irritation started about 2 weeks ago. Pagets was mentioned. A bit worried.

Hi @grammy2be, Breast MRIs are done in the prone position because the quality of the results when done on the back is not very good. A wide bore MRI machine allows plenty of wedges and pillows to be used so that you can be in a prone position while feeling like you are tilting upward rather than lying on a flat board. There are also FONAR upright standing/sitting machines which can do the job but are not that common. It might be worth traveling if there is one you can get to. The wide bore machines allow much more flexibility in positioning so the techs/radiologist might be able to figure something out using one of them. Sometimes sedation allows people who are very uncomfortable on their stomach manage to do it for the duration of the MRI.
It's definitely worth getting a well focused MRI since your symptoms could be Paget's. All the best.
