Request for Opinion on Adjuvant Chemotherapy Strategy
I am reaching out to seek your opinion regarding the adjuvant treatment plan proposed by two different oncologists.
By way of background, my mother is 68 years old and has already undergone breast-conserving surgery for a left breast lesion. The primary mass was removed successfully. During surgery, additional suspicious uptake was identified, and 10 frozen samples were taken, including 4 lymph nodes and 6 fibro-fatty tissue samples.
Final pathology demonstrated:
- Invasive lobular carcinoma with lymphovascular invasion
- Sentinel axillary lymph node positive for metastatic deposits with extracapsular extension
- Remaining lymph nodes showed only reactive changes
- Superior, medial, and inferior margins were reported clear
Additional tumor biology markers:
- ER (Estrogen Receptor): strongly positive (8/8)
- PR (Progesterone Receptor): strongly positive (8/8)
- HER2: negative (1+)
- Ki-67 proliferation index: 10%
The right breast was also biopsied due to suspicion on imaging, but pathology was negative. A full-body PET scan performed prior to surgery showed uptake limited to the left breast lesion and the involved axillary lymph node, with no evidence of distant disease.
Our current dilemma relates primarily to the chemotherapy strategy being recommended.
Regimen 1 – TC Regimen
- Docetaxel 120 mg
- Cyclophosphamide 1000 mg
- Every 3 weeks
- Total: 4 cycles
Regimen 2 – Dose-Dense AC → Paclitaxel
Phase 1:
- Adriamycin (Doxorubicin) 90 mg
- Cyclophosphamide 900 mg
- Every 2 weeks
- Total: 4 cycles
Followed by Phase 2:
- Paclitaxel 120 mg
- Weekly
- Total: 12 doses
In both approaches, radiation therapy and long-term endocrine therapy are planned afterward.
Our key question is whether, given her biology and staging, we can feel reasonably confident proceeding with the TC regimen considering its established efficacy and comparatively lower toxicity profile versus the AC-T approach.

Very sorry that your mother has to go through this. What actual stage is she, e.g. Stage IIIA, Stage IIB, etc.? I think the more advanced the stage, I'd go with the AC-T regimen, but I am no expert whatsoever. You could get a third opinion from another specialist and then go with the one that two of the three recommend? Just some thoughts here. I wish your mother and you all the best.
