Jean, thanks for the good thoughts! Yes, having the hematoma was traumatic but it did force me to slow down and take it easy. I was sleeping in a recliner after my first surgery and stupidly I would put the chair into recline position at night because I was too proud to ask for a little assistance. That certainly changed after the 2nd surgery because I ws then too weak to do anything! Glad I'm smarter this time around!

Congratulations, MBJ. I can totally understand how pleased you must be as there was so much anticipation. Hard to explain the high level of sheer relief and exhilaration to "others". Bravo!

Whitedove: Thank you so much. Relief and exhilaration is exactly how I feel. Not completely whole yet, but even the thought of having the ablilty to soon wear a normal bra or to not have to disguise myself with clothing or use prosthetics is just huge! They just feel like natural breasts to me, really tender breasts, but breasts all the same. Congratulations to both of us and to all of you having your exchanges this week, it is worth the wait! Yippee Squishees for all of us!!!

Congratulations MBJ. I was almost as excited to find out about your outcome as I was with mine. Funny! While I can't see the pics, just knowing you are happy with your "girls" is what's important.
Whitedove - I hope you are doing well. Are you in a regular bra yet?
Cleo - Best wishes on your surgery tomorrow. I too had a hematoma after surgery. It through me for a loop. No shock though.

MBJ - YAY!!!!!!!! I'm so happy for you. I can't get on the picture site either, but from all I hear ... you look great! When I woke up in recovery, I couldn't keep my hands off of mine ... I kept saying over and over, "They're so soft." I think the nurses thought I was a nut! LOL! I can't stop smiling ... so happy for you!

Thanks Giselle and deekay. I can't get enough to eat tonight! I'm ravenous, raiding the cupboards and refrig relentlessly! The idea of no food or water after midnight for an afternoon surgery bothers me! First question I asked at pre-op was if they planned to feed me when I awoke!

OK everyone, I am going to have a little rant right now, so feel free to look away-
So I saw my PS today 9 weeks after exchange and talked to her because I am having a lot of rippling. She said it's because my skin is so thin and there's nothing she can do. She said the ripples are the actual implants underneath the skin. To demonstrate she picked up a silicone implant and held it up by the end and you could see all the rippling down the sides. She said implants were designed for augmentation where they're covered up with breast tissue not for reconstruction. She said even though she used more Alloderm on me the rippling can still occur and can be permanent. Even though I understood her explanation I was still upset because this was the first I was hearing about this. She said maybe in a year she could go back in and add more Alloderm and it might (might?) work but she's never done that before, and it would be one more surgery and she didn't even know if insurance would cover it. Then she couldn't understand why I was upset. She actually said to me, "I should show you some pictures of some other women who have been really disfigured by their MX and you will feel so much better about your results." I was speechless at her insensitivity. So what I don't understand is when someone goes in for augmentation they're all concerned with the aesthetics afterwards and that's ok. But because we've had breast cancer we're not allowed to care about how we look and should just be ever so grateful to be alive? They voluntarily chose to have augmentation. We never chose this. I'm not as upset about the rippling as I am about the fact I was never told that if it happened it could be permanent because of my thin skin. Just one more thing to add to my imaginary book I'm writing in my head called "1001 Things Your Surgeon Will Forget To Tell You About When You Have Breast Cancer". (There is also a sequel called "1001 Things Your Own Surgeon Doesn't Know About Breast Cancer".)

Kate 33 - I would love to read your book and I could add some things too! #2 behind the rippling would be the whole lymphedema issue. But ... I hear you about the rippling. When I was waiting in pre-op my PS came and started drawing and telling me that I would have rippling and he showed me all the places that he thought it was going to happen. I remember thinking, "Why are you telling me about this now?" He seemed absolutely convinced it was going to happen. That's wasn't the best time to be telling me about it. I too have thin skin. My swelling has gone down a lot on my right, but I'm still pretty swollen on the left and the rippling is becoming more apparent. I'm sorry. Not what you needed today. If it is any encouragement, I've been doing a lot of research on 3-d tattoos and have seen a fair share of reconstructed breasts over the past week ... and I've been kind of shocked at how scarred up and mutilated some of these women look. It's just so sad. So, if it is any encouragement at all, ripples alone may not be so bad.

Funny reading about everyone so excited after their exchange.... I was disappointed and crying that they were too low and hanging too much (of course, my PS doesn't believe in bras).
Kate - When I twice followed up with him and cried more, his comment to me was, well you never had boobs before, right? Then he said well, if you're not happy, we can always take them out and then try again in 2-3 years. How about that for insensitivity? In fact, just got my reports and it's even in there... black and white. Well, I'm deciding between 2 other PS and I'll see a new one tomorrow... wish me luck. Thanks to everyone on here, I have a much better understanding of everything and have lots of questions.
Cleo -- I too had my mx on Jan 5th. Good luck tomorrow.

Kate - Oh, did you ask your PS if the alloderm was on top or bottom. I have a Cosmatrix ? sling that goes across the bottom.

vanderlady- She did the Alloderm sling when she placed the TE's and then additional Alloderm around the entire breast during exchange. I have no problem about how she did her job. I think she did everything possible. It was the feeling that I wasn't allowed to be upset about the final outcome, not even for a second, because I had BC.

Cleo: It's so funny that you posted that about raiding your fridge & pantry. I'm sitting here with a bag of M&M's and a huge glass of water as I count down to midnight. I have an afternoon surgery, too. Somehow it doesn't seem fair that we are held to the midnight cutoff regardless of the time of our surgery.
Kate: I'm sorry to read about your rippling and about your insensitive PS. I'll buy your book if you write it. Please include a chapter on what a lie it is when your doctor tells you that if you do such-and-such, you'll be done. "You'll have a lumpectomy, and then you'll be done." Followed by, "We'll do a re-excision, then rads and you'll be done." Followed by "We didn't get good margins, so we'll do an MX with no rads, then you'll be done." Followed by "Oh, we forgot to mention, just 5 years of tamoxifen and then you'll be done."

MBJ, if I come even close to how good you look after exchange, I will be delighted. I'm going to have to go back and read through all your deliberations. My PS seems intent on doing the Mx side first then doing the natural side at a later stage, and it seems he's aiming a bit low in the volume stakes - so there's plenty of negotiation to happen yet. hope the tenderness is settling down.

Kate -- sorry to hear about your rippling and the exchange with your PS. You have every right to be upset and you shouldn't let the PS discourage you. She sounds a lot like my (former) PS. I was so upset about rippling and asymmetry I could hardly stand it but when I went in for my last office visit, PS told me I looked great. I told him I was not happy and he reacted as though I was being ungrateful. He reminded me that they are "completely fake!" ... like a what-did-you-expect? attitide. If I hadn't already gone to another PS for a 2nd opinion I would have been devastated. He also told me that insurance would not cover a revision because it was not necessary. The new PS didn't criticize the work but thought a revision would help. I'm going in next week to replace the foobs with larger, silicone implants (currently have saline which ripples badly) and she's going to try to get them on the same plane and the same size. My insurance company didn't reject the preauthorization request. I don't know how this is all going to turn out but I am not ready to give up. In fact, if I couldn't find someone to help me I would have probably started over with a flap procedure instead of implants -- still might if the revision doesn't help.

First: Almagetty & Cleo: Best of luck with your exchanges and may your PS's do a beautiful job! Almagetty--if you are staying up past midnight I just replied to your PM. Sleep in late both of you and I hope you ate enough! My exchange was supposed to be in the morning and ended up being closer to afternoon so I wished I would have ate more! I was not hungry coming out of surgery though, just incredibly thirsty.
Kate: Wow, I am so sorry you are having problems with rippling! I have seen that kind of heartless attitude time and again and they act all indignant because we should just be happy we survived BCand feel we should be happy with what we do get, but I truly disagree. The whole point of plastic surgery is to return us as best they can to what we had before or close to it. I am of the school: the squeaky wheel gets the grease and I would keep on if not with this surgeon then another to correct the rippling (DawnHope you too!). I found out today that my dr. just did the fat grafting free of charge but MediCal Insurance doesn't cover it because there isn't substantial "evidence" that fat grafting works. Well, I think there is plenty of evidence that it does and it is going to take alot of very outspoken women to change insurance companies point of views! Okay, that's my rant! We are all here for you. Deborah, any advice?

Aw Kate so sorry you had such a lousy meeting with your PS. You know: this is just ONE doctor and she (he?) did a good job saving your nipples. Maybe now you need to consult with a PS who specializes in fat grafting? My understanding is that others have had satisfying results in diminishing the rippliing and divot issues with fat grafts.

Deekay: I just got back on a little while ago. Thank you so much--so far so good, just a couple of tweaks and they may not be able to repair the under arm area and return it to normal.

Kate: I am with Lilah! BTW: Did you say you used just surgical tape for your scars that were not laying flat? I had a very slight "dog ear" at the end of my incision line that I had my PS tape down but I am not sure this will work.

Kate - Has your rippling become more noticeable as the weeks have passed since your exchange? We both mentioned this issue a while back and I was curious if you were seeing more than before. I don't understand why these types of issues are not discussed in the physicians office BEFORE any surgery. I do understand that augmentation is a different animal from reconstruction. Thin skin is a big problem for us BC girls. Do any of you know if skin thickness can be measured prior to surgery? I wasn't told about the problem till after the TE surgery.

Kate you looked great in your pictures. Does the rippling happen when you bend over or are they there all the time?
I also thought that my PS should do some fat grafting but he doesnt think that it has great results and said that we will talk about it more after I have my nipples. I also thought that this would be covered with my medical insurance... If this is not the case then i will not be having that doen.. I have to say my implants really softenned up within the past few days and they look really good. They are far more perkier than before I had my MX.. I am just starting to appreciate what I have and be glad that I dont have to worry about the cancer.

jkz - I agree with what you just said and I am happy that you are softening up. Ripples do show up when I bend down as well. I have been practicing how to present myself during certain "activities", if you know what I mean.

Kate - sorry about the rippling - afraid I will get it too - PS mentioned something about it at my last appointment. He did say they could do fat grafting. I haven't had my exchange yet - so we will see I guess. One more thing to worry about.
