Obesity going into DIEP/Reconstruction

I am currently undergoing chemotherapy. I will have a bilateral mastectomy then reconstruction with DIEP. I am not sure I understand the timeline, it seems there is alot of dancing around regarding my weight. I am currently 41 BMI. Due to loss of insurance and gaining new I have not been able to talk to either surgeon. I am looking for information of anyone else's experience that was obese and went through this and a possible rough timeline of your experience. My job is physical and I am beginning to think I will have to completely change careers, I am extremely overwhelmed right now.

Can you try a consult with PRMA? Alternatively, if you are planning weight loss with any of the glp-1 meds, maybe discuss with your plastic surgeon whether having a tissue expander or implant in place until you reach a healthy weight, then go forward with DIEP. It seems like there is conflicting data on the safety and outcome, but at least with a center like PRMA they have experience with patients in this position.
