Anyone on low dose Enhertu?

Hi all,

I’ve completed 12 infusions of Enhertu + Perjeta for stage IV HER2-positive breast cancer. My last scan, and Signatera were negative and my tumor markers are in the normal range so I’m currently considered NEAD (no evidence of active disease).

Due to side effects like ongoing weight loss and loss of appetite, I’m considering lowering my Enhertu dose from 4.4 mg to 3.2 mg. Started at 5.4 mg.

Wondering if anyone here has stayed NEAD after reducing to 3.2 mg?

    First, congratulations on reaching NEAD! That’s very encouraging news. It’s completely understandable to think about quality of life when you’re dealing with ongoing side effects like weight loss and appetite changes.

    Dose adjustments are a fairly common topic with many treatments, and some people do end up lowering doses because of side effects. That said, experiences can vary quite a bit from person to person. What works well for one person may be different for someone else, especially in the metastatic setting where doctors often try to balance keeping the cancer controlled with making treatment as tolerable as possible.

    You might hear from members here who have had dose reductions on Enhertu or other HER2-targeted therapies and can share what their experience has been like. Just keep in mind that individual stories can’t really predict how things will go for someone else.

    If you haven’t already, it could also help to have a detailed conversation with your oncology team about your concerns. They may be able to talk through the reasoning behind different dose levels, how they monitor people after a reduction, and what options exist for managing side effects like appetite and weight loss.
