Inluriyo (Imlunestrant) with Verzenio (Abemaciclib)
Hi all. I haven't seen anyone post with experience taking imlunestrant in combination with abemaciclib. I know this is a newer treatment among many for HR+/HER2- individuals who have moved on from the first line. My first line treatment was Kisqali (ribociclib) with anastrozole for 3 years ending December 2025 and then I moved to a clinical trial for palazestrant (OP-1250) 60mg with atirmociclib which seemed to work initially and then stopped working within 4 months by April 2026 (this was a dose escalation phase 1b and my theory is the dose was too low) so I had to move on from it but I think palazestrant is a great drug and I wish I could have stayed on it. FDA approval projected in 2027 I think.
Progression is in my liver but my numerous bone and lung tumors have been stable since my first line treatment started in 2022. Following the palazestrant clinical trial, I was switched to fulvestrant with abemaciclib (standard of care) while I waited for my liver biopsy/ Foundation One and had 2 fulvestrant injection sessions then switched to imlunestrant 200mg 1x with abemaciclib 150mg 2x daily in June after my results showed the liver lesions have the ESR-1 mutation. I've been on this combo for 5.5 weeks.
I am tolerating the full dose of both meds well - the initial weeks included GI-issues, fatigue and general malaise which I think was largely abemaciclib because I was experiencing that prior to adding imlunestrant which has similar side effects. Side effects seemed to stabilize recently. I just had scans on July 15 and my liver is stable with minor shrinkage. I am grateful to have 3 months to breath before my next scan. I am looking into histotripsy for these liver lesions now that I know the systemic treatment is working.
Anyone else taking imlunestrant and/or this combination with abemaciclib and has experience to share?

I too am on Verzenio and Imlunestrant. I just started last week. I am on day 5 today.

I am an early riser, so I take my Verzenio at 6am with breakfast. Then, I take the Imlunestrant at 8am, as it is supposed to be taken on an empty stomach. Then I take the 2nd dose of Verzenio at 6pm. I have found a pattern of diarrhea/cramping 3-4 hours after the Imlunestrant dose. Maybe the Imlunestrant is the culprit, or them together. I am not having diarrhea issues of an evening after the pm dose of Verzenio.
You said that your GI issues resolved after a few weeks? I see you said you are 5 weeks in. I hope that is the case with me, as my body gets used to the meds. It has been a learning curve, as is the case with every new treatment, but I am tired of being in the bathroom every afternoon.

@candy-678 , I hope the GI issue will get better as you adapt to the new meds. I read through the Verzenio thread and adopted some tips from there….kefir in the morning and kimchi/sauerkraut at lunch which has helped moderate the severity of the diarrhea. I also take half of a multi-symptom imodium tablet as needed which used to be at least once a day and now it's once a week. I think most of the improvement is from getting used to these meds. I take Verzenio at 9am and imlunestrant and Verzenio at 9pm in the evening (2 hours after eating dinner even though I could take it 1 hour after). The pharmacist told me to take imlunestrant at night before bed initially to help cope with nausea but I haven't had nausea on it. I haven't switched up the schedule either.
