Finished with exchange surgery. Recieved silicone inplants on May 10th,2011. . I am not sure what size I am but the lady who fit me in a supportive bra said they look to be a full C or small D. I was a 34 DD before bc surgery & wore a Wacoal bra minimizer. I am so happy to have breasts once again. They are not even yet. Dr says it will take 3 months for the right one to come down.It is very high.(ugh) Nurse gave me message exercises to help this. Hope it works !
I forgot to mention this. I have a nipple on the left side, but none on the right.Dr thought I would like that if he saved the one nipple... Seems so abnormal to me,however I guess like everything else I will get used to it.

Would any of you ladies care to chime in on how long your PS expects your implants to last? A while ago, I had a PS tell me 10 years, and most everything I read says not to consider them lifetime devices. However, my own PS told me last week that today's implants will "outlive us". Well, I'm planning to live a long, long time Godwilling, and so, I'm not really sure what that answer means.

My plastic surgeon has been warning me that I could have my incision open up on the radiated side. He says that he had a patient have it open up after two months. When it happened did they go in right away and remove the implant and put the TE back in? It seems strange to me that a TE would not cause the same problem. Are you going to have to have one of the other procedures done instead? Tram flap, etc? My surgeon says he hates working with radiated skin. He mumbled something a week before surgery about not wanting to work with radiated skin anymore. I talked with my breast cancer surgeon about my radiated skin concerns and she said that I had a good plastic surgeon that could handle it. I hope it all works out well. I have a lot of people praying for me. If it does not I may look into a different plastic surgeon. I have meet someone in my support group that had problems and had some work done and had good results from her plastic surgeon. My plastic surgeon says he does not do some of the surgeries any more because he does not get reimbursed enough from insurance.( the one were they have to attach blood vessels) He said I would have to go to a research or university hospital because a lot of PS are not willing to work for what the insurance will reimburse them for.

hmmm, I had BMX in March. My right had been previous radiated (13 years ago with lumpectomy and chemo) and I was under the impression that I had to have some sort of extra support for my right side. For me, the recommended procedure was a lateral dorsi flap (think that's the right terminology). In my humble opinion, he doesn't sound too positive about the whole thing. What does everyone else think?0

LOL Tina I know, I know!!! Im trying! We went grocery shopping today but I lifted or reached for NOTHING and I didnt do the unloading or putting away. Other than that, I dont move out of my chair! I'm trying!!!
Edited to add: My son and husband were with me so I didnt pickup anything more than a yogurt cup...mainly I handed out coupons for them to fetch.

Since I'm finally healed and into expansions I've started asking my PS all the TE, implant, and exchange questions I've been storing up for him. I asked if there's anything I should be considering now during the expansion phase that will affect my final outcome. I specifically asked able ripples. He said that he doesn't hear complaints about rippling issues from his silicone implant patients, but he does from his saline implant patients. I've been thrilled with him so far so I really want to believe him.
From what I've read here rippling with silicone implants sounds like the norm and dependent on skin, radiation history, etc. Does anyone know how much surgical/technical expertise comes into play???

Sue - this is scaring me...the BMX was quite easy for me. E-day is approaching and I keep thinking i will bounce back and be back to work in a few days and back exercising. Ughh. This whole things keeps going on and on and on....

From what little I know, implants have a higher failure rate on radiated skin, and my PS says that risk does NOT diminish with time. I imagine this refers to capsular contracture VS the incision site opening up (but I don't know). Personally, I am not considering flap surgery at this time, especially since I've seen some really good results with implants on the picture forum and in a book a friend loaned me. The only other option I am considering at this time is micro fat grafting, either alone or in combination with implants. I am going to visit a Dr in Miami who has pioneered micro fat grafting for whole breast recon to see if this is right for me. If implants don't work and I can't have MFG, I may consider flap surgery, but I'm not there yet.

Your PS's comments would not inspire confidence in me. Yes, we may have more complications due to the rads, but we deserve to get the best care possible, even if it means the PS is inconvenienced by this. Don't know where you live, or if you're willing to travel, but if you are considering flap surgery, there are a couple of very well-respected surgical groups who do this (New Orleans, charleston, Chicago). I spoke to one of them over the phone and he gave me a lot of good info. TRAM is an older procedure and a lot of PS no longer do it because of the risk of abdominal wall weakness. However, many PS are not trained in microsurgery, so they don't do the newer techniques. Have you checked out the NOLA and Charleston threads?0 -
Pinkbutterfly -- send a PM to Estepp... her first name is Laura and she is always happy to talk with others who have been radiated and are having implants. She falls in both categories. Also: re your question about life. They are required to say 10 years as I believe that is what the manufacturers promise. But as I understand it they can last longer (as much as 20 years). Also it is my understanding that one needs an MRI every 3 years to check and make sure the implant is not leaking. Since silicone -- especially the new silicone (gummy or not) which is more viscous -- does not leak out or get absorbed into the body like saline, only an MRI would detect damage. It doesn't hurt anyway to have an MRI every few years anyway (from a breast cancer perspective). But exchange is not as hard to recover from as BMX.
That said, ladies, it IS a good idea to take it easy post exchange
Most can and do go back to work within a few days or a week, even, depending on what sort of pocket work is done... but even with that you need to take it easy.My understanding re: when an implant fails is that if the implant has to come out due to infection then it is most common that nothing is put back until the infection (or the skin that has failed) is healed.

Well as a uni I still have to have a mammogram every year anyway.
The other nice thing about the 10 year shelf life is that you can benefit from advances (as they come) in the future. And whether you have silicone or saline there is a need to change out periodically... so implants are never permanent.

Sending lots of "big" thoughts to all you ladies having exchanges this week! For those who expressed a desire to be even bigger, talk to your PS; they can try for a larger implant on the OR table, yes?
Re: Implant replacement...I know several women who had augmentation between 15-20 years ago and have never had theirs replaced!

I know a woman who had saline implants 15 years ago but had to have them replaced. I think its just something that can happen... but I'm guessing that more often than not they last closer to 20 than 10 years.

Good Luck to everyone this week!!! YEAH!!! Ive been told its time to put my one week update photos out. I think my DH is enjoying the boob flashes (and camera flashes). LOL
Anniemom- you can do it!!! I am really surprised how little pain I have now. A little tenderness around the outsides of the pocket and bruising too, but the incision already look good, my arm pain (from the TE is gone) and I can poke around the implants without pain. You can do it!
And I promise I will continue to take it easy this week...not as easy as last week as my DH is working the rest of the week, but easier than usual for SURE!

Renn: When going for my US after chemo there was a woman in the waiting room with me who had hers in for about that long and her silcone implants had ruptured and were leaking. Though the new ones are better, and the gummies are even stronger then that, they still recommend changing them out every 10 years as a safety measure. I would be afraid to have mine in that long!

I had my exchange last Friday and am feeling pretty good. I didn't have much pain at all and managed with just regular tylenol. From what I can see, my foobs look pretty good in my surgical bra but still feel weird. There are some squishy spots and it is just hard to know what is swelling versus the implant etc. I have drains again and that is by far the worst of it. Only two this time thankfully but I don't see me getting rid of them anytime soon. I am happy I have drains since I think it probably helps you heal faster versus letting all the fluid sit in your body, but they are a pain for sure. I am supposed to travel to California on Monday and just hoping I can do it - will only go if I feel well enough and the drains are gone. Crossing my fingers.....
Good luck to all of of you with upcoming surgeries. I was dreading mine only because I didn't want to go back to all this cancer stuff and drains etc., but if I am closer to done it is worth it!

I had a drain on my implant side (could have probably used one on the reduction side since I had a bleed and a HUGE bruise). If I had ONLY had implant exchange I definitely would have been fine a week later. As it was, the healing for the reduction side took longer. I also for whatever reason got very low red blood count following that surgery (perhaps due to having had 4 in a calendar year) and so was extremely tired a week or more later... so much that I drove a week later and got very dizzy attending an event. My boyfriend was there so he drove us home (I wanted to drive to see if I could). This was due to low red blood count though.
BTW, did someone here ask what DH stands for? It stands for Dear Husband. I suppose I could write DB for Dear Boyfriend

BTW - one week photos are up. I dont see much change...but I do notice my incisions are turning flesh colored in spots...so that's good news!

so excited for all of you heading for exchange (sounds a bit like a sci-fi novel!) I can't stress enough the resting part....the taking care part....even the mind....it has been a helluza journey and you all deserve to rest up and let your mind body and soul heal...:)

Oh thank GOODness for Westieluv! With the new girls Ive got, DH is following me around the house copping feels every hour on the hour!!!! Why dont we all talk about playtime out from nipples, AND exchanges??? I didnt think to ask when I was coming around from anesthesia!!! LOL.
