First I should again qualify that my delayed recon was 22 years ago as I noted, so what was done then and what is done today may be different. No, I did not have an expander. As i recall, the skin sparing mx just ended up with excess skin in the area. It's not really formed into a breast. This is why I said if you do this and later decide to just stay flat, you might want another surgery to remove the excess skin.
I should also note that I don't know anything about keeping nipples. The first mx it was something that was rarely done. The 2nd mx, my tumor was too close to the nipple to be able to keep it.
So, on your options list, for me, the 1st time was delayed flap reconstruction (no expander).

Sorry you have had to join this not so exclusive 'club', but glad you have found us.
I had a lat dorsi flap as recon on one side. I had to google lat myocutaneous flap as I had not hear of that. It sounds like myocutanous flap is a SUBSET of the lat dorsi flap.
In your case, as I understand it, they would be doing the flap to repair damage done by radiation and you would still be flat? It sounds like the type of flap you are doing was kind of a predecessor to ps realizing they could use this for breast recon. I found this article in case it helps:
In my case, I was happy with the procedure and haven't had any lasting problems. My breast surgeon did set me up with some PT as soon as my PS cleared it just to be sure my muscles got completely healed.
I have heard of several people with issues as a result of their lat flap. Often, I think this is related to how experienced the ps is at doing flaps. So, the most important thing is to be sure that is the case.

Cindy - sending prayers for your niece. If it is pancreatitis that is nothing to fool around with. My brother developed it after gallstones and it took several months to clear up. Of course, he waited a very long time before he finally went to the ED. Hopefully your niece can be treated quickly.

Our pool is being removed this week. When the liner was removed we discovered the metal panels behind the liner are rusting and the PoolCrete floor is cracked. We are not surprised but it reinforces our decision to remove the pool. The cement patio on the bottom right of the picture below is cracked and crumbling. It is also being removed. They expect to finish by Thursday.

I allowed myself 35 minutes to get to the ENT office today for my annual check-up. Of course, they were doing road work on the only direct route with traffic down to one lane (2 lane road). So I did manage to get through quickly only to get to their only parking garage and the entrance is blocked by a very large tow truck. Unsure if it was wedged in the opening or as it turns, just blocking access. It finally moves after 10 minutes and here there was a major car accident on the second floor of the garage. A van and a large SUV had major damage to the front of the former and the rear of the latter. The office is located across the street on the 3rd floor so I had to then get from it to the office.
Finally get to the office just in time and have to complete this form that they want annually. They are not associated with the MLHS and have no access to the portal. However, they work under the aegis of the MLHS. Go figure how much confusion this causes. The form is full of duplication ,which drives me crazy with at least 2 sections on each page where they ask you to print your name and then to provide your signature! The receptionist told me that since it had be 12 months and 2 weeks since my last visit, I had to complete the form yet again even with no changes in the areas addressed. I didn't complete some sections because I hate useless duplication. All told they had 2 pages of computer printouts of my PMH to review, 5 pages of the repetitious print here, sign here pages and then another form of 2 pages. TMI in my estimation. Everything is fine and I see him in a year.
Decided to sort through the large bin of held mail and 98% of it was junk mail. I did get a few items that I need to deal with including a gyn bill that Medicare refused to pay for, meaning IBC won't pay either. For her to write a prescription for a skin medication, she billed me $150! I have never been billed for this in the past and if they insist I pay, I will say sayonara. It was always included in my prior exams. She was paid for a pelvic exam and a breast exam; I refuse the breast exam because I prefer it being done by my MO. I'll ask my new PCP to write for it in the future.
harley07, our next trip is a land tour of Ireland. We are combining 2 different tours even though it means we will have some overlap in the sites. That's okay, because we were frequent Ireland tourists, so we know the sites well and can manage on our own. I honeymooned there and my children were taken there as youngsters, as teens and as adults. We will be there for about 5 weeks total. We used to rent a car and go all over much to the chagrin of the car rental agencies when we would return the cars with thousands of miles on the odometer. Next fall is another cruise to Hawaii and then the Pacific islands south of there, including Fiji. Norway and Arctic cruise in fall 2027 if we are still vertical, lol.
Scheduled mammo for early December today so check list is getting shorter. I will see Pulmonary next week and hope to schedule sleep test soon after.
I also need to talk to Pain Management again concerning ongoing issues with back pain. It was present for the entire cruise, so I rule out stress as a causative factor, and feel it is more related to my steadily disintegrating spine. PA felt MRI might be indicated so I will see what she has to say. By the end of the day, I find it difficult to straighten my back and hate that I am slumped over.
Chris, what does the doctor want to do to your other thumb? Glad to hear your cast is off and the new orthotics will be available next week.
harley07, wow the pool removal is a big deal. Where will they get the fill? Was the patio you were referring to as collapsing the one with the pergola. Love the size of your backyard, it is huge.
cindyny, hope your niece gets a dx and tx plan soon. You are such a support for your family and friends and still manage to be the energizer bunny between visits. I used to have energy like that, sigh. Hope your packing for FL is just about finished and you can relax on your way home.
Sandy, hope Bob is still okay and that he can get his new glasses soon. Hopefully, you will finish your unpacking before Thanksgiving so you can then enjoy and relax in your "new" home. Funny how quickly the cats settled in.
carole, I admire how regimented you are about your exercise routines.
After the doctor's appointment today, I ran to Produce Junction to restock the refrigerator and was planning on making meat loaf for dinner. I had DH pull meat from freezer but didn't get home until after 1 and had other things that needed to be addressed first. So meat loaf will be on the menu for tomorrow. I also stopped at Sherwin Williams to pick up some paint chip samples for the dining room. I had to sent the rug out for cleaning, so now is the ideal time to empty the china closet and have DB paint the ceiling and walls. I can do the woodwork. I also want to find someone to remove the drapes for dry cleaning who can then also rehang them.

Yup. Got the cast cut off yesterday. Then went down to the OT department and had a lightweight splint made. Had a late lunch, then showed up for podiatry early, but they called me in early. I have to wait a week for new orthotics. In the meantime she put some padding down by the tarsal metatarsal joint of the little toe. I looked that up. Parallel to a certain thumb joint.
Went home, A friend and I thought there was a meeting at church but not. My Visible told me to take it easy today. Doh!! I got through it without getting super tired. Still doing space bar with forefinger.
This is my last week with bike. Won't be getting it out unless in an emergency after this week, temps below freezing every night. 😪 Doctor and I discussed left thumb. Still don't know for sure. Wearing a splint on that hand all the time, too.
Glad for the negotiations on SNAP, but I could have squeaked by on Advantage Plan groceries and pantry.
Busy writing again, massive Bible study project. So glad for the cross-referencing and footnotes of Catholic Bible. We don't use a Concordance.
Meds safe expected today. I won't be keeping it in such an obvious place. So dumb!!
