Question about Sentinel biopsy results

I had my lumpectomy with Sentinel node biopsy on Nov 14 and reviewed the pathology report with my doctor today.

He said 5 nodes were removed during biopsy and 2 are confirmed to have cancer >2mm. Based on this, he advised chemo is required and ALND is on the table but he is going to review with the cancer board at the hospital.

Wondering what are others experience with treatment plan when in the same situation. Did you also have ALND. I have stage 2 HR/PR +, HER2-.

    Hi @baffled26, I was told that for stage 2 ALND is recommended for 3 or more positive nodes. Has an Oncotype DX test been done on your tumor? The results of this genomic test recommend whether chemo should be done. I had a lumpectomy for stage 2 HR+/HER2- cancer with one positive node so no ALND. Because of the positive node axillary radiation was added to the whole breast rads planned. My Oncotype score was low enough that chemo was not recommended since it would not be very effective.

    I live in the US now but previously lived in Ontario. In my experience medical protocols were very similar but I was not treated for cancer there. Ask your doctor why he recommends ALND and chemo. I hope the rest of your treatment goes well whatever is decided.

    Thanks so much @maggie15. I didn't get an oncotype test yet. This doctor is my surgeon and he just referred me today to the oncologist. He said the oncologist will make final determination on chemo so I will be sure to ask for the test.

    For ALND he said because the size of cancer found in the lymph nodes is greater than 2mm. If it was smaller, he would be OK with radiation but based on my age he may want to do the surgery. He did also say he wants to review with other surgeons and oncologists at the cancer board to make a decision as a group.

    I just would like to be prepared to ask more questions and or get second opinion if needed so the info you provided is super helpful.
