    I guess I don't understand ... on my ipad ... all my PM's are there when I log on ... I got the link on my ipad from going back and looking the PM that was sent to me from nowheregirl and linking it from there.

     I can't help rebetata on the botox ... I don't know anything about it ... someone will, though.

    Whippet mom -- i would love your advice.   I have Allergan 133Mv - 400 cc TE in.  The PS filled me a lot at surgery.  I liked my profile and projection when i was at 300 cc, earlier this week.  They said OK, then we fill you one more time (i.e. 360) because in order to be the approximate size I was at 300cc, they overfill one more time beyond.   With this latest fill, i feel too big.  I had hoped to end up closer to a B cup-- but 360 feels like a large C small D to me.   any thoughts?  do they really have to overfill one size to get to a smaller size the the exchange.   I may go in next week and ask for saline to be removed.  I feel like i have rocks now-- and i didn't feel that way last week.   any thougths?  is it Ok to have saline removed?  thanks!!

    Thanks for your thoughts Kayb -   FYI-- i have loads of extra skin, in fact, i will need lots of cutting and tucking during my exchange surgery.  I was a 34DDD...  I just don't want to end up with big breasts again at the end of this.  I'd rather be smaller than i want than larger than i want.    I'm not in more pain per se from this fill (though i still have very bad ROM and am starting PT) -- it's just that this latest fill expanded the TE in a way that the breast-- which feels like a rock-- skims my arms when they are at my side and drives me crazy.  it's just adding to the claustrophobic feeling of having these fake hard things inside of me. 

    I think i'm having a meltdown today.  who knows.  could be hormones.  also just loads going on in our life.  but i am such a perfectionist by nature and having to "choose" my breast size and then counting on a PS to make it look that way, is just way too much for me right now.

    Kayb -  yes, my PS is on board with my plan and, i think, clear about my wishes.  he has been very good about saying he'd do whatever i want in terms of size.  he told me not to think about cup size per se-- but just to let him know during expansion when i liked the projection etc..--  i liked it BEFORE this last fill.  but he said that in order to achieve the look/projection of that-- he needed to overfill because the muscle snaps back as soon as the TE is removed.  i guess i need to let go and trust..but am just bumming for some reason.  i'm not having exchange surgery until October-- so i have to live with this for a while.  

    I am 5'6 and @ 138 lb -- not sure what my chest wall size is.   so did you actually get filled to 475cc, or is that just what the TE accomodates?  so interesting that you are only a c with 475.  it's all so confusing.  but your post is encouraging. thanks!

    YES! it is far away :(  but i can't do it this summer because we are going away end of august and PS doesn't think he should do it before i travel.  And then september comes and life gets crazy with  kids starting school (and my daughter is starting new school). and so we enter October!

    500 cc...and you are pretty tiny from your stats.  that must have been interesting :)  but it sounds like you are happy now, which is great!!

    So you are a 475 c cup?   so it sounds like maybe my 300cc (ish) may indeed be a "b"ish cup??

    I had a uniMX Feb 23 with TE placement at that time.  My TE is a Natrelle Style 133LV-14 and I am now up to 500 ccs.  Earlier Whippetmom gave me a recommendation of high profile 600-700 ccs.  Does that mean I need to fill the TE to that same amount?   I know we are not to go by cup size but I ususally wear a 38C and told the PS that I don't want to be smaller than that.   I am 5'8 and have a long torso so I don't want to look out of proportion.  If I am going to have to go thru this why not end up the size you want? 

    I have the feeling that he would rather go smaller, that it would be easier in gaining symmetry.  I have scheduled exchange surgery tentatively for July 5th.   I don't know if that allows enough time or if it will need to be rescheduled.    He tells me everything looks good, should I be confident in what he suggests or be less than happy with the end results?

    longislandmom:  I don't know where they come up with this nonsense, that they need to fill you to a certain amount to get you into an implant which will make you look the way that you did at 300 ccs in a 400 cc volume tissue expander?  HUH? 

    Here is the schtick: You have a 133 MV-400.  Your TE has a width of 13.0 cm.  So they need to use an implant which has as close to, the same as or greater than the width of your TEs.  You are not being "overfilled" at all - you have not even reached the full capacity of your TEs.  So you are going to need at least 475 ccs in a high profile smooth round silicone implant, or 397 ccs in a midrange plus profile smooth round silicone implant.  With the latter, you will have quite a bit less projection than you have with the TEs, but women always say they love the projection of the TEs and it is nearly impossible to replicate that with implants.  I suppose that the Allergan Style 45 can come close, but that is too much volume for you with the width you need.  So there are your two sizes.  If you start looking at anything less than 12.7 cm, you end up with too much pocket width and too narrow an implant and then they want to migrate laterally, towards the armpits.  Additionally, rippling is an issue.  So it is best to have a nice snug pocket and approximate the width of the TE as closely as possible.

    longislandmom:  Reading further, with your comment about having a lot of skin - you are going to fare better with a high profile implant.  You would need 500 ccs as a minimum and perhaps you could use 650 ccs in an Allergan Style 45.  You will NEVER be as large as you were pre-BMX.  I don't understand why they did not excise the skin at the time of the BMX.  They are doing this at the time of the exchange?  Now I can perfectly well understand why you do not need to fill to capacity.

    longislandmom - I can certainly understand that claustrophobic feeling of the TEs!

    I didn't discover this thread until after my surgery, and my TE's had already been placed. I got some big honkin' ones, I'll tell ya. (I was rather large, and the original girls were 38DDD). I did tell the PS I wanted to go smaller, and he wanted to make sure I got something that was proportionate to my body size.

    When Whippetmom first instructed me to measure my chest, it was 39". What I didn't realize was that a lot of that was from the post BMX swelling.

    Then I started losing weight, and as I got smaller, the TE's appeared to be larger and harder. Now it's impossible to sleep on my side at all.

    And - now my chest measures 33", and my PS will have his work cut out for him at exchange. I have 35 more pounds to lose, so there will be lotsa nippin' and tuckin' goin' on!

    Whippetmom - I am laughing at your comment about women loving the projection of their TEs. Projection? I'll tell you where mine project - under my armpits!!!! And up to my collarbone!!!!! Seriously, my foob starts exactly midline under my armpit. I don't think it started out that way, but I was a lot fluffier when I started....Wink I am thinking I will be having LOTS of pocket work done at exchange. But that's o.k. At least that might spare me a revision down the line, exchanging out stripper boobs for normal-sized ones.

    Longislandmom : Now, I look like a B cup. I want to look like a C cup. I think I will have to be fill up to 450 or 500 to have that look...Outch !!! I will discuss with my PS tomorrow morning. Before BMX I was a saggy B cup and before brestfeeding 2 child I was full C/small D so I think I will enjoy be a full B/small C ! I am 5'5'' @ 120 lb and my ribcage 29''.

    @ Whippetmom - not sure why they didn't take off more skin at BMX-- I wish they had! Maybe the MSKCC assembly line had to move forward..lol...but my skin does NOT need expanding right now, i have plenty!  I had a very good mtg w PS last week who assured me it will all be taken care of. I'll have even longer scars -- but i guess that comes with the territory of having such large breasts to begin with 

    Truth be told, as long as my breasts are not too big-- i don't care exactly what size they are (i.e. so long as they end up small C or under).  Are the size implants you are discussing @ that size?  I am very happy with the way my TEs sit/are positioned on my chest.  they look very symetrical  and the placement is lovely. Only drag is the ugly extra skin -- but that's not visible in clothing.

    @Blessings -- sorry about your rocks!  i am not sleeping on my side yet either.  Good luck with your weight loss! what kind of program are you on?

    @loulou-- i can relate to the post-breast feeding sag! though my breasts remained large!  just became thinner and saggier.   Funny, when i was explaining what was going on to my kids, my 8 year old son's biggest concern was what would happen if i got pregnant again-- how would i feed the baby?  lol.  i explained at 48 i was not having any more babies.  but not to worry, we could always use a bottle!

    Suggestions please?

    Had BMX last year. Then went for recon in February. Had 100cc in there at that time. Got 120cc at my first fill in march then 60cc each time after that. Up to 460cc in each side now. The left is coming along nicely, rather soft to palpate. The right is hard, scar tissue around the TE. The right is also where i had rads back in 2004.

    Usually I have some upper back ache for a day or two after fills that with motrin and gentle pressure applied to the area are relieved.

    PS wants to go to 600-650 to allow pocket work and a 500cc implant at exchange.

    At this point I'm not sure I'll get there. If the next fill is like this last thursday, I'm thinking we may be close to doing only what my body will allow. Anyone else had this happen and what to do?

    Well of course the TEs are horrible rocks (aka coconuts and turtles) and the actual implants are very soft by comparison and, in most cases, stay where they are supposed to be (and if they don't there is the magic word "revision").

    For those of you still in TEs: they often look stupid and sit in the wrong place, just get reassurance from your PS that this will be corrected at exchange; they are hard and uncomfortable, especially as time goes on and they get more filled; the final implant will not project as much but will be softer!

    Also with TE's most of us had pain near the end. I mostly had 50cc fills, one 75cc fill that nearly killed me and my last two fills were 25cc's. If you're hurting, ask for a smaller fill. Also muscle relaxers, I took Flexeril, others take Valium, but that was the only thing that relieved my pain near the end. Ask your PS for muscle relaxers if you don't have any.

    Lilah - I mentioned to my PS that I'd had some nice compliments from ladies on a picture forum I belong to, and that they noticed my TEs were well-placed with nice symmetry.

    His answer? "Just wait til you see the finished product!"

    We laughed about that...he's not braggy at all, but shyly admits that he does get really good outcomes.

    He's about 12 years old and reminds me of Doogie Howser. But he has the best reputation around for his delicate, precise work.

    whippetmom- You picture of where to measure was so helpful. I measured my rib cage and I am at 29 inches. I have a small built and am 5'4". I never really fit into a 34b without it having padding. My ps said she was thinking about a 250 for my exchange. I have no idea what size that will be. When this all started she asked if I wanted to be bigger and I said, hell ya. With this information can you tell me about what size bra I would take? I am only at 170 cc's for my fills so far. I have 350 expanders in.

    I saw my PS for pre op ths week. He says that he will need to do my cut under my breast because my skin is thin and "shinny". He is worried about the skin. Is there any problem with this that I should be aware of?

    mebmarj: I personally think you need fat grafting to the previously irradiated breast to soften that skin and allow it to heal.  There is a thread on that topic and I will bump it up for you - in the "Radiation" forums...

    Also, are you massaging with oil to soften that skin?
