debb: In theory, you would need at least 475 ccs in a high profile smooth round silicone implant, or 375 ccs in a moderate plus/midrange profile implant. However, I personally feel that you could use at least 13.0 plus cm in width, which would translate to a 500 cc or 550 cc HP or a 450 cc moderate plus profile implant. It depends also on how you are feeling about the size you have currently with your TEs. Are you happy with the size? Do you like the projection or would you feel better with an implant which had less projection? But this gives you some idea where you would be.

Thank you guys for replying back to me . The pain is better but I sure am sore today better then last nite. That is my last fill thank the Lord. I will go to the ps office Monday to have someone check it out. I can breathe better today. GOD BLESS YOU ALL GOOD NIGHT. I NEED THE SLEEP!

OK-- question for all of you reconstruction ladies...i am going to have my first fill on wednesday. I'm a bit stressed by it because i'm not in any real pain now and am dreading it. any advice? how much pain were you in? should i take painkiller before? after? valium? thoughts?? tx!

Hi long island mom,
I was quite small prior to my mastectomy. My first fill was 70ccs, because that's what my PS suggested. It did leave me in some pain, but everyone is sooo different in this regard. Some women can easily tolerate 100cc's or more with no discomfort at all. The actual fill itself was not painful for me, just the stretching afterwards. Anyway, for me the key was to go slower. I ended up doing 50cc's every two weeks rather than trying to do 70 every week, and that was a breeze. I think the first one is the hardest because we just don't know what to expect. I didn't take a pain pill prior, because I was driving, however I did take a muscle relaxer (prescribed by my PS). That helped a lot! Good luck, you'll do great!0 -
Longislandmom my PS expanded me about 60CCs each time (so the nurse told me). I was in no pain for the first 6. The last 3 were painful for that day but I found taking an Aleve at the office relieved that. Do note that I took the Aleve for the last 3 expansions. This was after chemo ended and long after surgery. (They don't do expansions during chemo at my treatment center). If you are just a few weeks out from surgery be sure they will let you take Aleve.0

My TE pain actually went away once I started fills.
Now I'm done with fills at 620ccs, and losing weight....there's less of me and more of them, making it hard to sleep on my sides.

I had 390cc my first fill and 230cc my second and didn't feel a thing. I have 60 more to go on my way to 720cc.

Your worst thoughts will probably not occur or will be incorrect as were mine. I had 120 ccs at time of surgery and proceeded with 70 ccs, every week for 3 weeks, then went down to 60 ccs for 2 weeks. I am getting close to being done with the fills, I am not sure how much PS plans to go but I am scheduled for exchange surgery Jul 5th.
The fill it self has been pain free for me, the needle they use to fill is very small I just look away and they tell me how many ccs they have put in. He says if I feel pain I should tell them, but I have never felt anything but a little uncomfortable. I have occassionally taken Ibuprofen for the first day/night. But absolutely have found it to be quite tolerable. He said he doesn't want to go too fast, so if at all possible just keep them at small fills.
I see some have had large fills such as Mooleen, I don't know if that is what her PS generally does.

Those are some mighty big fills is that the way he usually goes with patients? Have you had any discomfort with them being that amount?

Deborah - thank you soooo much for your help in determining what size to go - I think it worked out perfect. And when I saw my PS a couple of weeks ago - he said they had softened even more - so I guess the drop and fluff can happen for quite a while after. He is so pleased with the results, as am I. Just wanted to know I appreciated you help.

I had implants already so my theory is my muscle was already stretched some. Also the PS said my muscle is not in very good shape so it probably stretched easily. Also I had DD, so I have a lot of skin.

longislandmom i had 6 fills with no pain at all except the day after my last fill and that was not bad at all. It just felt tight for a day or two. But for me it was easy breezy. I did not take anything, I did not even need the numbing cream for the needle.0

catalog number is 16659619 and 16985311. I have an appointment with the ps on Wednesday. I really like the size now and don't want to be any larger. I know she will want to expand me more because there is some rippling on my right side. ...Any advice. I am at 350 cc at the moment, which sounds kind of low in comparrison to many people on this board. I'd like to be a C cup. thanks for helping!

traceyb - I was filled to 310cc's and received 325 cc implants. I felt HUGE with 310cc's in my TE's but was disappointed with how much smaller the 325 cc's implants were comparatively ... didn't have near the projection with the implants that I had with the TE's. If you like the size you are now with your TE's ... you need more cc's in an implant to get to the size you are now with your TE's. (the TE's are rigid .... so that have 'form' and structure and projection that you don't have an implant ... that is why you need more cc's in the implant).0

Tracey, unfortunately, the numbers you provided do not give me the information I need. See if you can get the style number and the volume. It might be something like this.: MX13 or MV14. You might need to call your doctor's office for this info.

Tracey: here is the issue...
You are filled to 350 ccs, but you might have a TE which has a fill volume of 400 ccs, 500 ccs or 600 ccs. They all have different dimensions, and it is the dimensions we need.0 -

Its been awhile but I was reading up on your thread here and saw the news about Sientra. I am wondering if this will be a good option for me? I am not sure if you remember my measurements but I can repost if you need.

Rebecca: Of course I remember! You have 800 ccs in Style 45. Do you not feel they are large enough? The Sientra implants would be a good option for you. They do not have a extra full projection style, so you would gain some width and volume, but not projection. You probably could use some width to boost up your cup size. Ask your PS if she can request a visit from the Sientra rep.

I will do that sounds good. I need the width especially on the left side where I had the first initial lumpectomies. You memory is amazing as ever . I was also waiting for the auth. from Kaiser for a botox injection into the pectoral muscle have you ever heard of this being done? It is to relax it a bit as it tends to push my imlant around and smash it?

Can I ask a question? How about using the Sientra implant along with fat grafting, would that give a more fuller breast with better projection?

whippetmom my PS is waffeling about if I need fat grafting. Can that be used to create more upper pole fullness? My results are very nice and I am pretty happy so I think he is right to think we probabaly dont need it now. but I am not sure. As they have settled they look better and better so I am glad we had to wait before we do the final surgery. I am also trying to lose weight 5 down 25 to go.

I have had the fat grafting twice so I am hoping that it will give me the foulness I am looking for. I have a wide frame but the fat grafting I had in the front near the new nip helped me a lot with the conical shape that is lost in the recon.

Fighter: No, fat grafting really will not add projection. It is not used for that purpose. It is used for contour defects in the upper pole, rippling, lumpectomy defects. In Rebecca's case, it is added to create more dimension to the nipple/areola complex, but it will not make up for the significant loss in projection between Allergan and Sientra. I think it might be 3 centimeters - something like that.

Carrol: It can be used to ease the step off between the chest and the implant and thereby can create the illusion of upper pole fullness. But for the type of FGT you are referring to - it will not do what might be accomplished with the type of FGT performed by Dr. Khouri and his associates. It would take many FGT sessions to create noteworthy upper pole fullness. But if you are happy, do nothing. Just buy pretty bras and enjoy! Avoid general surgery whenever possible.

Thanks for the reply whippetmom. I am only thinking about this because I still ahve my nipples surgery ahead and my PS will be doing some lipo to my sides. So I wanted to get anything else I may need done all at the same time. I ma hoping when I lose weight I will like the result better, my body image is so messed up now because I feel fat and gross all the time. My cleavage looks really good though. So I guess I will decide 25 pounds from now what else I may need.
